RDRO Telescope Authorization Agreement
By signing this form below, you agree that you are fully responsible for the telescope, CCD camera, and eyepieces that you are being given access to. This means that from the time you sign out the keys until you return them to Diane Fujitani, you agree to be liable for any damage or loss suffered by the telescope, CCD camera or eyepieces, unless such damage is due to natural events or events beyond your control (such as earthquakes or crash landings of unidentified flying objects).

If any damage happens during your use of the equipment, or is discovered when you first arrive at the telescope, you agree to report it immediately, in person or by e-mail to Lewis Kotredes (ltk@astro.caltech.edu, 219 Downs, x6634). It is in your interest to report such damage promptly.

By signing below you also testify that you have been checked out by either Lewis Kotredes, that you have received a copy of the operations manual, and that you feel confident to use the telescope in a competent manner.

If you have not been checked out on the operation of the CCD camera, you agree not to use it.
